Our Practice Nurses are passionate about Preventative Health and, along with your DOT GP, offer a range of services to help you stay well and live well, including:

Health Assessments

  • a once-off 45-49 year old Health Check for people aged 45-49 years who are at risk of developing chronic disease
  • a Type 2 Diabetes Risk Evaluation for people aged 40-49 years with a high risk of developing type 2 diabetes as determined by the Australian Type 2 Diabetes Risk Assessment Tool
  • an annual Health Assessment for people aged 75 years and older combined with your Drivers Licence renewal medical (win-win – thorough check-up AND your licence renewed in one appointment!)
  • an annual Health Assessment for people with an intellectual disability
  • an annual ATSI Health Check for people of Aboriginal or Torres Strait Islander origin – this also allows 5 extra subsidised allied health appointments
  • an annual Menopause Health Assessment to support ladies experiencing peri-menopause or menopause.

Healthy Heart Checks

From age 45 or from 30 years for Aboriginal & Torres Strait Islander patients.
An assessment of your risk of heart disease (heart attack or stroke over the next 5 years) and to identify any lifestyle modification that is recommended.
Thirty minutes is all it takes!

Chronic Condition Management Plans

Previously known as GP Management Plans (GPMPs) or Care Plans, these are designed to support patients with ongoing health conditions such as diabetes, asthma, heart disease, arthritis, and many others.

Care Plans involve regular reviews with your GP and practice nurse every 3 months, focusing on helping you manage your condition and achieve your personal health goals. They also provide access to Medicare-subsidised allied health services, including physiotherapy, dietetics, and podiatry.

These plans are an excellent way to stay on top of your health — ensuring prescriptions, referrals, and important screening tests are always up to date, while giving you structured, ongoing support.

Weight Loss support

With and without injectables like Wegovy & Mounjaro. Our nurses provide support, accountability and education as regularly as you wish to help with your weight loss. You may also be eligible for various Health Assessments and a Care Plan to assist accessing a dietitian or exercise physiologist. You have your own cheer squad to celebrate your wins!
