Previously known as GP Management Plans (GPMPs) or Care Plans, these are designed to support patients with ongoing health conditions such as diabetes, asthma, heart disease, arthritis, and many others.
Care Plans involve regular reviews with your GP and practice nurse every 3 months, focusing on helping you manage your condition and achieve your personal health goals. They also provide access to Medicare-subsidised allied health services, including physiotherapy, dietetics, and podiatry.
These plans are an excellent way to stay on top of your health — ensuring prescriptions, referrals, and important screening tests are always up to date, while giving you structured, ongoing support.
With and without injectables like Wegovy & Mounjaro. Our nurses provide support, accountability and education as regularly as you wish to help with your weight loss. You may also be eligible for various Health Assessments and a Care Plan to assist accessing a dietitian or exercise physiologist. You have your own cheer squad to celebrate your wins!
